[FX.php List] Bad HTML being generated from FX (Still)

Hi John,

What is contained in these included files?
require_once ('server.php');
require_once ('includes.php'); // some dev utilities not used in  
production

Are either server.php or includes.php writing out the invalid hidden  
form elements?
