[FX.php List] Uploading files

Thanks for the help.

I have the submit page working using the coding David Ness provided.   
However, I am not very good at the processing page.

I get the processing page to run but I do not find an uploaded file.   I am 
testing it on the localhost.

In OS10.4 made a folder in the Library/WebServer/Documents.
