[FX.php List] Bad HTML being generated from FX (Still)

Something is writing out mal-formed hidden inputs between your <body>  
opening tag and the top of your <form>. By the looks of them, the  
source of the data is $_POST. (It even has Google Analytics values.)  
So, what code between <body> and <form> would be echoing out the  
contents of $_POST as hidden form fields?
