[FX.php List] What can possibly be wrong?

Incidentally, I also echoed back my IP address & port number, and  
they echoed back properly...

All of my expected fields from my form echo back as well. This is so  
strange... I keep looking for the typical syntax error, but I can't  
find one...

Permissions are set on the db, it opens remotely as it should...
