[FX.php List] Error Code 102 and I don't know why

Is there any kind of validation on the field that could cause it to fail
when creating throught the web.

Have any of you ever had a 102 "Field is Missing" error when creating a
new record even though you are absolutely certain the field is there?
I've commented out everything to troubleshoot and I still get the error.
Has anyone else had this?
