[FX.php List] Why doesn't this parse?

Phew!  That did it.  New "Case" has been added to demonstrate.

I realize that this is not perfect. I am trying to work my way out of 
having to create and send mass emails for clients after getting them 
started with online data collection.  This is a first step.  I will 
be looking at Smarty as a next step and SwiftMailer instead of 
PHPMailer.
